This Notice describes how information
about you may be used and disclosed and how you can get access
to this information. Please review it carefully. You have the

Premier will file your insurance
for you and follow-up with your insurance company(s) for sixty (60)
days. After sixty days you will be responsible for any remaining
balance. You are responsible for payment regardless of your insurance
coverage or legal representation in situations such as accidents
or workers compensation.
– Please provide current copies of your insurance card(s)
and notify us of changes immediately. We also need your co-pay,
deductible and co-insurance information. If you have no insurance,
we will be working with you to communicate the financial aspects
of your care.
– It is your responsibility to obtain all required referrals
and authorization approvals from your insurance company. Failure
to do so will result in reduced benefits and higher patient responsibility
for you.
– All co-pays, deductibles or co-insurance are due at the
time of service.
- If surgery is required, you will be responsible for all or a portion
of the fee. We will forward your information to our financial counseling
department, verify your benefits and contact you to discuss an estimate
of your financial responsibility. If you are having non-urgent or
elective surgery, 100% of the payment will be due prior to providing
the service. If you have a life threatening illness and need urgent
or emergency care, we will not delay your surgery however, non-urgent
or elective surgeries may be delayed pending payment of your responsibility.
– You must contact your employer, workers compensation insurance
company and be assigned a case worker before we will be able to
treat you for a workman’s compensation injury.
– Payment options include cash, check, MasterCard, Visa and
CareCredit. Installment Payment Contracts are also available for
balances <$1,000. We ask that balances over $1,000 either be
paid in full or placed on a credit card or CareCredit. For billing

CareCredit applications are available at www.carecredit.com.
– Balances that remain unpaid for 30 days are subject to interest
at 18% APR. Balances that are turned over to collection agencies
are assessed a 30% collection fee.
